Data Collection Form

 

The form below is aimed at collecting data from incidents, concerns, and observations from Bearhawk owners and operators. It does not replace normal incident/accident reporting if a reportable incident or accident has occurred.

The data collected will be catalogued over time to observe if similar events are occurring on multiple aircraft, and if they are then a Maintenance Note or Safety Note will be added to the for others to read.
